Has anyone ever received their $25 back?
when i sent mine this year I put note on it that I never got last years reimbursement last year. got a letter back and said it was submitted as a medical claim and not as a wellness claim. I can't find it in the instructions how the doctor should fie the claim

Just don't pay the copay at time of visit and tell the doctor to submit it to insurance. Never rely on the company to reimburse you. It's just not a priority

I agree they could make it easier on us with this issue.

I've found it easier to go to a Concentra clinic and be done with it.

Concentra is not all over the country. As I mentioned before, I have done this wellness screening several times, and when they ask for the co-pay at the doctor (there is no concentra in my area) I just tell them to submit the copay amount with the billing for the visit. I would never pay for it and trust that Saia would reimburse me. That isn't in my job description.
