WVA.R& L Rolled the tail pup.

This crash was 2 semi's 1 set of R & L Double pups,and a van.
One of the drivers was killed.
I've done a lot of searching and have found one report with the fatal drivers name,but still don't know which company he was driving for.
Here what I've got so far.
