DOT physical form

Has anyone printed the DOT physical form off UPSers? I looked at it and it has the whole long form, but i dont see any medical card where you can cut it out. Is that still supposed to be on there or has it changed or what?

we still get the card from the service ctr .but have heard doc might have them

I got mine from the DR. that did the physical.

Remember That when we renew our CDL's, we have to take the LONG FORM to the DMV. No long form ,no renew!

the fmcsa.gov has the medical form with card that you can download/print.

I just did mine three days ago. I went to the UPSers website and used the link to print out the form. I only printed the pages that needed to be filled out, 3 of them. The Doctors office had the card. The UPSer website says that the site to fill out the form online to print is down, but it has been down for months from what I hear. You just have to print the PDF form from the site and fill it out by hand.

Got it all figured out. Our HR lady has the medical cards.

I mean HR has blank medical cards for the doctor to fill out at the physical.

You can fax a copy of your med card to dmv. You don't have to go in person.
