I don't believe that the DOT has a regulation concerning flat soled shoes or sneakers. However, your employer may have work rules requiring boots or steel toes.
The eyeglass question is also a good one and I believe that you only have to have proof of prescription glasses or contact lenses. Personally, I wear contacts and I always carry a spare pair of lenses with me. I also carry a pair of glasses for emergency purposes. The glasses also come in handy when driving at night and my eyes get bleary from concentrating.
Anyway, sorry I couldn't really answer your questions, but I'm sure someone else will have the facts.
