If you are going for a company physical they may give you one to take with you. I'll share this story to save anyone else the trouble I went through once upon a time:
FedEx manager says hey Space, go to Dr. Dipstick and get your physical, take this form with you and make sure he fills in all the info. Upon my arrival at Dr. Dip I state "FedEx wants this form filled in". Yeah, we know, we do this all the time he says. 3 hours later I return to manager with manila envelope sealed. He opens and says I have to take it back and get Dr. Dip to fill in the FEDEX FORM, not the Dr's form! You've got to be kidding me, he didn't fill it out?! I drive clear back across town in rush hour traffic (on my own time) to have the Dr. fill out the FedEx form I told him needed to be filled in the first time.
I was somewhat HOT upon arrival. When I got to the receptionist desk, there was a line 137ft long. I waited patiently on the outside while on the inside my blood pressure was at nuke 1000 psi. About 45 minutes later I got my turn at the wire reinforced window with the little half moon opening at the bottom. I calmly explained what I needed and she took it back to the Dr...... 35 minutes later she comes back and says "Dr Dipstick has already left and is on vacation for 2 weeks". I tried to fit through the little half moon window, but couldn't get past my shoulders. I went into cardiac arrest. Called FedEx manager and he said no physical, no driver. Mine expired in 2 days. About 9 a.m. on my first unpaid day off I drove back up to Dr.'s office and threw down a break bad come to Jesus hissy fit. I had the FedEx form filled out and another Dr's signature 3 minutes later. grrrrrrrrrrrrrrrrrrrrrrrrrrrrrr
So moral of the story: If company gives you a form to take with you, stand over Dr's shoulder and watch until the last i is dotted. Aaaahhhhhhhhh, I feel so much better getting that all out!!!
