Could someone tell me how to find out what is on your DAK report? I'd appreciate all the help I can get. Thanks

you are entitled to one free copy per year, more free copies if you are denied a job.

i may be misunderstanding something here, so i will ask....'

are you guys trying to obtain all your personal information from DAC online...??

this ain't going to happen. you need to send them photo copies of your drivers license, and your Social security card....

then snail-mail it to DAC/USIS...its probably about 2 weeks time from when you mail it to the time you recieve it.

but if you just wanted the website for thier address then ok.........

Thanks for your help Pro1driver. I went to the website and printed the form to obtain my report. I filled everything out and sealed it an envelope. It didnt say anything about photo copies. So I didnt include any. Our company is shutting down operations in 4 of our terminals and putting about 350 drivers out of work in 60 days. I basically want a copy of my DAC report to see what if anything is on there before I start looking for a job. Thanks Again.
