then scroll down to "consumers"...
go down the page and look for the link to obtain a free copy, and print up that form
fill it out completely and be sure to also get a photo copy of the drivers license AND social security card..
in the "check boxes", make sure "free yearly report" is checked, we are allowed at least 1 free copy per year....then mail it ASAP
now when the DAC/USIS report comes in, go over it closely..
then if anything isn't true or he wishes to "dispute it", go back to the home page of DAC, same steps as before, now scroll down to "dispute form"... print it out, fill that out, license and social security card photo copies are NEEDED AGAIN, and write down what is the dispute. then mail it ASAP
let DAC do its job, they will send him another free copy with his "dispute/rebuttle" response.
