DARIEN - -- Two rear wheels came off a tractor-trailer traveling on I-95 on Sunday night and rolled into the oncoming lanes, where they killed a Bronx woman who was a passenger in a minivan.
A 2006 Kenworth tractor eastbound near Exit 9 when a mechanical failure caused two left-side wheels to pull off the axle, state police determined. The wheels hit the westbound minivan and a westbound tractor-trailer.
